Job summary

The Affinity Hospice Management LLC in Connecticut is seeking a Hospice Aide who will work under the supervision of a registered nurse and perform tasks per the patient’s plan of care to promote comfort and meet personal needs.

The aide is responsible for reporting and documenting observations and care performed, with duties assigned to promote quality, continuity and safety of the patient's care. The role supports caregivers and integrates with the care team.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understand hospice philosophy and care for the terminally ill.
• 1 year experience in hospice, orderly, acute, rehabilitative, or long-term care preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family with ethical, respectful care and confidentiality.
• Ensure safety and operate equipment properly as directed.
• Assist with baths, back rubs, oral hygiene, shampoos and bed linen changes.
• Assist in dressing/undressing and feeding as assigned.
• Provide reminders for self-administered medications as allowed by state regulations.
• Report pain or changes in patient condition to the RN Case Manager.
• Assist in ambulation and transfers as instructed by the nurse or therapist.
• Keep living area clean and orderly; adhere to care procedures and documentation standards.

Education

Certified Nursing Assistant (CNA) or state-approved equivalent training

Nursing Assistant Certified (NAC)

Current Healthcare Provider CPR

Current Food Handlers Permit

High school diploma or GED
