Job summary

Affinity Health Management is seeking a hospice aide to provide compassionate in-home and facility-based care under an RN's supervision. The role emphasizes comfort, safety, and thorough documentation of observations and care performed.

You will work with patients and families, perform personal care, assist with activities of daily living, and report changes promptly while maintaining confidentiality and respect.

Qualifications

• Completion of a state-approved certified nursing assistant course.
• NAC certification and CPR certification required.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Hospice philosophy understanding and comfort in terminally ill care.
• Prior hospice/long-term care experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Provide personal care and assist with activities of daily living as assigned.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment; observe and report changes.
• Assist with bathing, grooming, feeding, and linen changes.
• Aide in dressing, positioning, and mobility as directed.
• Assist with medication reminders where allowed by state rules.
• Maintain cleanliness of living area and complete light housekeeping tasks.
• Report concerns to RN Case Manager promptly.
• Assist with bedpan/urinal use and light laundry tasks.
• Document observations and care given in EMR per procedures.
• Provide respite support to patient/family as appropriate.
• Adhere to professional conduct and organizational procedures.
• Participate in quality improvement activities as required.
