Job summary

Affinity Hospice Management LLC is seeking a hospice aide to work under RN supervision, delivering compassionate personal care to patients and supporting families. You will report observations, document care, and help promote comfort while maintaining safety and dignity in the patient’s home or facility.

The role emphasizes clear communication, adherence to care plans, and a proactive approach to safeguarding patients and supporting the care team.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• NAC certification and CPR certification are required.
• Current Food Handler's permit and a valid driver license are required.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminally ill patients.
• 1 year of hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Provide personal care and attend to patient requests promptly as assigned.
• Communicate with patients and families with empathy and confidentiality.
• Ensure safety and proper use of equipment during care.
• Assist with bathing, grooming, oral hygiene, and linen changes.
• Assist in dressing, feeding, and mobility as directed by the RN.
• Monitor patient condition and report changes to the RN Case Manager.
• Assist with light housekeeping and transport needs as required.
• Maintain clean, orderly living areas and proper documentation in EMR.
