Job summary

Affinity Hospice Management LLC in Danbury, CT seeks a compassionate Hospice Aide to support patient care under RN supervision. You will assist with personal care, safety, and comfort per the plan of care, and report observations and care performed.

You will communicate with patients and families, maintain confidentiality, and document outcomes. A CNA (NAC) credential and CPR are required, with prior hospice/home-health experience preferred.

Qualifications

• State-approved CNA course completion.
• NAC certification.
• Current CPR certification (hands-on).
• Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions.
• Understanding hospice philosophy and terminally ill care.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care after assignment; report to Case Manager if unable.
• Provide patient and family with compassionate communication and confidentiality.
• Maintain patient safety and use equipment properly.
• Assist with baths, oral hygiene, shampoos, and changing bed linens.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding; do not provide homemade foods.
• Monitor skin integrity to prevent tissue breakdown.
• Report pain or changes to the RN Case Manager.
• Assist in ambulation and exercise as instructed.
• Provide ROM and transfers/positioning as directed.
• Assist with medication reminders where allowed by state regs.
• Assist with bedpans/urinals and light housekeeping.
• Documentcare and participate in quality improvement activities.
• Keep living area clean and orderly; respect patient/family.
• As a mandatory reporter, report non-compliance within 14 days.
• Report suspected neglect/abuse to proper authorities.
