Job summary

Qualifications

• Active RN license in the state of practice.
• Willingness to travel and bring care to patients' homes.
• Current BLS certification.
• Ability to pass state & federal background checks.
• Professional liability insurance is required.
• Strong problem-solving and compassionate, empathetic approach.

Responsibilities

• Conduct thorough patient assessments and provide personalized, comprehensive in-home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate care with patients’ plans, approved hours, and preferences.
• Monitor patient symptoms and report changes promptly to the care team.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and collaborating physicians for a unified care approach.
• Maintain accurate patient documentation in the EMR.
• Provide emotional support to patients and families.
