Qualifications

• Active RN license in the practice state.
• Willingness to travel to patients' homes.
• Graduate from an accredited nursing program.
• Current BLS certification.
• Ability to pass state & federal background check.
• Malpractice insurance is required.

Responsibilities

• Conduct thorough patient assessments and deliver personalized, comprehensive care.
• Educate patients and families on health maintenance and disease prevention.
• Provide ADL assistance with compassion and respect.
• Coordinate schedules with patients and align with care plans and preferences.
• Monitor and report changes in patient condition to appropriate providers.
• Administer medications and treatments as prescribed.
• Communicate with Case Managers and physicians to unify care.
• Maintain accurate patient documentation in the EMR system.
• Offer emotional support to patients and families.
