Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Physical Therapist to evaluate and provide therapy to patients as prescribed by physicians. You will collaborate with physicians and home health staff to plan and implement care aligned with patient goals.

Qualified candidates hold a California license, CPR certification, and a valid driver’s license with auto insurance. Some institutional experience is preferred; travel to patient residences is required.

Qualifications

• Licensed Physical Therapist in California.
• CPR certification required.
• Driver's license and auto insurance required.
• Preference of at least 1 year of institutional experience.

Responsibilities

• Partner with the physician in evaluating patients and establishing goals based on function.
• Instruct patients in prescribed therapeutic activities to relieve pain and restore function.
• Record and report patient reactions and changes to the physician and home health staff.
• Train patients and caregivers in use of prosthetic, orthotic, and assistive devices.
• Provide client total physical therapy program and hand out instructional content.
• Prepare assessment and plan of care, write progress notes, and document in patient record.
• Prepare periodic evaluations and discharge summaries.
• Follow Plan of Care and communicate with care providers.
• Notify agency of absences affecting services.
• Participate in discharge planning and complete discharge summaries.
