Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Each visit lasts about 45 minutes and uses an iPad-based clinical workflow; physicians are not required to prescribe medications or order labs.

Active, unrestricted MD or DO licenses in the coverage area are required, with ability to travel locally within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to cover the following primary locations: Erie - Region

Responsibilities

• Provide in-home health evaluations for patients.
• Engage with patients in their homes for health assessment and education.
• Use iPad and clinical workflow; no prescribing or lab orders required.
• Travel locally within licensed states; conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated by health plan.
