Job summary

Signify Health is seeking a Per Diem Physician to provide in-home health evaluations. The role involves visiting members in their homes, performing medical reviews, and conducting physical evaluations, sometimes including additional diagnostic tests as requested by health plans.

This is a part-time, per diem opportunity with local and interstate travel. Clinicians will use an iPad with a clinical workflow and are not required to prescribe or modify treatments.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

• Collaborate with Signify Health as a clinician colleague in the specified role.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services such as spirometry, bone density screening, etc.
