Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide in-home health evaluations. You will visit patients in their homes, review medical histories, perform physical assessments, and deliver diagnostic services as indicated by their health plans.

Role offers part-time, per diem hours with an hourly base rate, malpractice coverage during Signify Health activities, and supplies. Travel within licensed states and virtual visits may be required.

Qualifications

• Active, unrestricted license in the coverage area.
• Multi-state licenses preferred.
• MD or DO physician qualification.

Responsibilities

• Conduct in-home health evaluations for ~45 minutes.
• Review medication history and perform physical evaluation.
• Travel locally within licensed states; conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as ordered by health plan.
• Join a national network of clinicians to improve health outcomes.
• Engage with patients in-home to bridge gaps in care.
