Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to perform In-Home Health Evaluations for members in their homes. The role involves medication and medical history review, physical assessment, and using an iPad workflow to document findings.

You will travel locally to visit patients and may perform virtual visits as needed, across licensed states, with a base hourly rate and incentives per visit.

Qualifications

• Active, unrestricted medical license in the coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Visit patients in their homes for In-Home Health Evaluations and health assessments.
• Coordinate with health plans to connect patients with appropriate care.
• Travel locally and to licensed states as needed; conduct virtual visits when required.
