Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication history review, a physical evaluation, and optional diagnostic tests ordered by the health plan. You will use an iPad with a clinical workflow and may travel locally.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations during ~45-minute visits.
• Collect medication history and medical history; perform physical evaluations.
• Coordinate with health plans to arrange diagnostic tests as needed.
• Travel locally within licensed states; conduct virtual visits as required.
