Matrix Medical Network is seeking a licensed NP to perform comprehensive health assessments in patients’ homes or SNFs nationwide. You will review medical history, medications, and risk factors to inform care decisions and improve outcomes.

Responsibilities include conducting geriatric assessments, coordinating with PCPs, and ordering relevant tests while ensuring clear documentation and adherence to Matrix guidelines. The role offers flexible scheduling and potential growth opportunities.

Qualifications

• Master’s Degree or commensurate NP experience with licensure.
• Current NP licensure in state of practice.
• Board certification in Adult, Gerontology, Family, or related NP specialty.
• BLS/ACLS/CPR certification required.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance as required.
• Strong computer skills; familiarity with health/medical record software.

Responsabilités

• Perform adult/geriatric assessments, including history, diagnoses, and referrals.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Coordinate education and follow-up with PCPs.
• Provide services at patients’ homes or Skilled Nursing Facilities.
