Job summary

Matrix Medical Network seeks a licensed NP to perform comprehensive in-home health assessments across the nation. You will review medical history, medications, and social factors to identify care gaps and guide treatment plans during visits at patients’ homes or SNFs.

The role offers flexibility in scheduling, competitive pay based on assessments completed, and opportunities for licensure support and continuing education. Travel up to state lines is expected.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescription authority.
• AANP, AACN, or ANCC board certification in an NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Travel up to 80-90% with potential overnight travel.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and documentation.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facilities).
