Job summary
Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical histories, medications, and living environment to identify care gaps and coordinate follow-up with PCPs and care teams.
Responsibilities include directing point-of-care testing, collaborating with physicians and social supports, and delivering patient education. Roles offer flexibility and potential transitions to full-time positions, with sign-on incentives
Qualifications
- Master’s degree in nursing or equivalent experience and NP licensure.
- Current NP licensure in state of practice with prescriptive authority if needed.
- Board certification in Adult/AGPC, Family or related specialty.
- BLS/ACLS certification and valid driver's license.
Responsibilities
- Conduct adult/geriatric health assessments, review medical history and documentation.
- Administer and order point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, and other care team members.
- Provide care at members’ homes or skilled nursing facilities as required.
- Ensure clear documentation per Matrix guidelines and protocols.
