Job summary

Matrix Medical Network delivers in-home comprehensive health assessments to older adults and at-risk individuals nationwide. As a full-time NP clinician, you conduct visits at a member’s home or facility, reviewing health history, medication adherence, and risk factors to close care gaps.

Qualifications

• Master's NP degree and current NP licensure in the state of practice.
• Board certification in Adult/Adult Gerontology or Family NP specialties.
• Current BLS/ACLS or CPR certification and valid driver's license.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis and referrals.
• Administer and order point-of-care and lab-based testing.
• Collaborate with physicians, case managers, social workers and caregivers.
• Provide services at the member's residence (home or SNF).
• Educate patients and perform follow-up as needed.
