Job summary

Matrix Medical Network conducts comprehensive in-home health assessments to evaluate medical history, current treatments, and potential gaps in care. Providers perform geriatric assessments, order tests, and coordinate with physicians and case managers to deliver high-quality, patient-centered care.

Visits take place at the member's residence, including homes or skilled nursing facilities, with flexible scheduling and ongoing professional development supported by onboarding and preceptorship.

Qualifications

• Master’s degree in nursing and NP licensure in state of practice.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care NP.
• Current BLS, ACLS or CPR certification.
• Valid driver’s license and auto insurance in residence state.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Perform comprehensive health assessments in the member's home.
• Review medical history, medications, and adherence.
• Collaborate with PCP and care team to coordinate care.
• Order and administer point-of-care and lab tests as needed.
• Document findings per Matrix guidelines and protocols.
