Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments across our national network. You will review health history, medications, and social factors to improve care quality and identify undiagnosed conditions.

Responsibilities include conducting geriatric assessments, ordering tests, collaborating with physicians and care teams, and providing services at patients' homes or SNFs.

Qualifications

• Must hold NP licensure in state of practice.
• Board certification in Adult, Gerontology, Family or Acute Care specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment plans, and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and other care team members.
• Provide in-home health assessments at member residences (home or SNF).
