Job summary

Matrix Medical Network conducts in-home comprehensive health assessments to review a member’s health, history, medication adherence, and social factors. Assessments help identify care gaps and guide next steps, with visits typically lasting up to one hour.

Providers work with PCPs, care managers, and family/support networks, visiting members at home or in SNFs. The role emphasizes empathy, clinical accuracy, and coordination across teams.

Qualifications

• Must hold NP licensure in state of practice
• Board certification in the listed NP specialties
• Current BLS/ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Valid state driver’s license
• Automobile insurance coverage with state-appropriate minimums
• Strong computer skills and familiarity with health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation according to Matrix guidelines and protocols
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members and other clinical staff as appropriate
• Coordinate with PCP on patient education and follow-up as necessary
• Provide services at the member’s residence which can be in homes or Skilled Nursing Facilities
