Job summary

Matrix Medical Network seeks skilled nurse practitioners to perform comprehensive in-home health assessments across the country. Providers review health history, medications, and social factors, delivering insights that help close care gaps on a single visit.

Qualifications

• Master’s degree or commensurate NP experience and active NP licensure.
• Current NP licensure in the state of practice with prescriptive authority if needed.
• Strong communication skills and computer literacy for health records.

Responsibilities

• Conduct adult and geriatric assessments including medical history, diagnosis, and follow-up.
• Administer point-of-care and lab testing as required.
• Collaborate with physicians, case managers and caregivers to coordinate care.
• Provide services at members’ residences, including homes or SNFs.
• Document findings clearly following Matrix guidelines.
