Job summary

Matrix Medical Network is seeking nurse practitioners to perform Comprehensive Health Assessments at members’ homes or facilities. Visits last up to one hour and may include medical history review, medication adherence, and risk assessment to improve care gaps.

Providers administer tests as needed, collaborate with PCPs, and work with care teams to educate patients. Flexible scheduling and competitive pay apply, with full-time opportunities and support for licensure and credentialing.

Qualifications

Master’s Degree or commensurate experience and NP licensure
Current NP licensure in state of practice with prescriptive authority or ability to obtain
AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
Current BLS, ACLS or CPR certification
1 year NP experience preferred; new grads encouraged to apply
Valid state driver’s license for the state of residence
Automobile insurance coverage for the state of residence
Strong computer skills and familiarity with health/medical record software
Excellent verbal and written communication skills

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation
Administer and order point of care and lab-based testing as needed
Collaborate with physicians, case managers, social workers, and caregivers as appropriate
Coordinate with PCP on patient education and follow-up
Provide services at the member’s residence (homes or Skilled Nursing Facilities)
