Zusammenfassung

Matrix Medical Network seeks NP providers to perform Comprehensive Health Assessments in members' homes, including medical history review, health education, and care coordination. Assessments take up to one hour and may occur in homes or Skilled Nursing Facilities.

Providers work with physicians and care teams, may travel to members’ residences, and benefit from flexible scheduling, competitive pay, and potential FT opportunities with sign-on bonuses.

Qualifikationen

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice, including prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult/Family/Gero specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for state of residence.
• Automobile insurance coverage requirements for state of residence.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.

Aufgaben

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatment, and referrals.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
