Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive health assessments in members' homes or SNFs. You will review medical history, medications, and risks, and collaborate with PCPs and care teams to close care gaps.

Responsibilities include assessments, testing, referrals, and clear documentation, with a flexible schedule spanning weekdays and weekends to meet member needs.

Qualificações

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in admissible NP specialty (Adult, AGPC, Family, etc).
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers for coordinated care.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
