Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments. Responsibilities include reviewing medical history, medications, and social factors to identify gaps in care.

The role requires active NP licensure with prescriptive authority, board certification, BLS/ACLS, and 1 year of NP experience. Expect extensive travel (80-90%), a supportive team, and competitive pay with a $7,500 sign-on bonus.

Qualifications

Master’s degree and NP licensure with current state license.
Current NP licensure and prescriptive authority.
AANP/AACN/ANCC board certification in eligible adult or family specialties.
BLS/ACLS certification and at least 1 year NP experience.
Eligible to travel 80–90% with overnight stays as needed.
Valid driver’s license and auto insurance; reliable transportation.
Strong computer skills and experience with health/medical records software.

Responsibilities

Conduct adult/geriatric assessments including history, diagnosis, treatment and education.
Administer point-of-care tests and select laboratory testing as needed.
Collaborate with physicians, case managers, social workers and caregivers.
Coordinate patient education and follow up with PCPs and teams.
Provide services at the member’s home or in Skilled Nursing Facilities.
