Qualifikationen

• Current NP licensure with prescriptive authority.
• Board certification in Adult, AGNP, Family, or related specialty.
• BLS, ACLS certification.
• Travel up to 15% and valid driver’s license.

Aufgaben

• Perform comprehensive health assessments in patients’ homes.
• Review health history, medications, and risks; document per Matrix guidelines.
• Administer point-of-care and lab testing as needed.
• Coordinate with PCPs, case managers, social workers, and family members.
