Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by experienced NPs. You will review members’ health, medical history, medication adherence, and social factors to identify care gaps during one-hour visits.

Responsibilities include performing geriatric assessments, ordering tests, coordinating with physicians and care teams, and delivering services at members’ homes or SNFs. Travel up to 80–90% with potential statewide or regional coverage is required.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in a relevant NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Able to travel up to 80-90% of the time with overnight travel.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, and referrals according to Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, caregivers, and other personnel as appropriate.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence, which can be in homes or Skilled Nursing Facilities.
