Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home assessments, reviewing health history, medications, and care gaps. Assessments are conducted in members' homes or SNFs, with care plans designed to improve outcomes.

Providers work flexible schedules, with opportunities for full-time roles and a monthly earnings potential based on assessments completed, plus licensure support and ongoing education resources.

Qualifications

• Master’s Degree or commensurate NP experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice including prescriptive authority or ability to obtain.

Responsibilities

• Perform adult/geriatric assessments including history, diagnosis, treatment, education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and other care team members.
• Provide services at member homes or Skilled Nursing Facilities as needed.
