Zusammenfassung

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or SNFs. You will review health histories, medications, and risk factors to identify gaps in care and coordinate with physicians and case managers.

This role offers flexible scheduling and potential FT opportunities with a competitive compensation model. Applicants should hold an NP license in the practice state, board certification, and current BLS/ACLS.

Qualifikationen

• Master’s Degree or commensurate NP experience with licensure.
• Current NP licensure in the state of practice and prescriptive authority as needed.
• Board certification in Adult/Adult Gerontology or Family/acute care specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged.

Aufgaben

• Conduct adult/geriatric health assessments including history, diagnosis and treatment planning.
• Administer or order point-of-care and lab testing per Matrix guidelines.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate patient education and follow-up with PCPs.
• Provide services in members' homes or Skilled Nursing Facilities.

Ausbildung

Master’s Degree or commensurate NP experience

Current NP licensure in state of practice

AANP/AACN/ANCC board certification

Current BLS/ACLS/CPR certification

1 year NP experience preferred

Valid state driver's license

Auto insurance coverage (state of residence)
