Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by qualified Nurse Practitioners. Assessments focus on medical history, current health status, and medication adherence, with documentation per Matrix guidelines.

The role supports care coordination and may involve referrals and follow-ups in patients' homes or SNFs. The position emphasizes flexible scheduling, the potential for full-time roles, and a $1,500 sign-on bonus.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice including prescriptive authority.
• AANP/AACN/ANCC board certification in relevant NP specialty.
• Current BLS, ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric health assessments in home or facility.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers and other staff for care coordination.
• Provide in-home health education and follow-up aligned with Matrix guidelines.
