Job summary

ACCESS is seeking a Housing Stabilization Case Manager for the SSVF program to assist Veterans and their families in achieving housing stability through wrap-around services.

The role involves in-office and community/home visits, caseload management, eligibility screening for federal/state programs, goal setting, crisis intervention, and referrals to ACCESS programs. This position reports to the SSVF Program Supervisor.

Qualifications

• At least 1 year of case management experience or similar experience working with participants.
• Ability to interact effectively with individuals from diverse backgrounds and maintain confidentiality.
• Excellent written and oral communication skills.

Responsibilities

• Maintain a minimum caseload of Veteran families and individuals as determined by ACCESS procedures.
• Interview and screen eligibility for Federal and State Assistance programs and other supports.
• Assist participants in setting short- and long-term goals to improve sustainability.
• Provide supportive, one-on-one interactions to build trust and positive relationships.
• Monitor progress and maintain participant files; ensure accurate data entry and reporting.
• Refer participants to other ACCESS programs and external resources as appropriate.
• Maintain accurate vouchers and reporting information; ensure timely case notes.
• Respond to homeless emergencies; provide crisis intervention with clinical support when needed.
