Job summary

Qualifications

• Bachelor’s degree in human services with 1–3 years of experience or substitution allowed.
• Experience in case management, connecting veterans to resources and housing.
• Knowledge of Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Strong written and oral communication; proficient with databases and computers.

Responsibilities

• Enroll participants rapidly through outreach with the SSVF team and partners.
• Assist with housing stabilization, rapid resolution and housing placement.
• Coordinate referrals to VA benefits, legal services, mental health, and other supports.
• Develop housing plans and progress notes; maintain documentation per guidelines.
• Travel within the greater Boston area for placements and housing follow-up.
