Job summary

Insight Housing seeks a Case Manager to support veterans and homeless families. Under supervision, you will provide housing stabilization services, coordinate referrals, and connect participants to VA benefits and community resources.

You will maintain client files, conduct home visits, and collaborate with Housing Specialists to develop action plans and budgets. Excellent communication and crisis intervention skills are essential.

Qualifications

• High school diploma required; BA in Social Services or related field a plus.
• CPR/First Aid training required.
• Experience serving homeless populations preferred.
• Proficient with HMIS data systems.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate SSVF intake and eligibility for new referrals.
• Provide outreach to homeless Veterans.
• Maintain a client caseload and conduct housing/stability services.
• Develop budgets and service plans with each Veteran client.
• Maintain and secure comprehensive case files and HMIS records.
