Job summary

Columbus House, Inc. is seeking an SSVF Case Manager to support veterans who are homeless or at risk of homelessness. The role includes housing search, placement, counseling, service coordination, and progress evaluation to achieve housing goals.

The position involves both site and mobile case management, collaborating with local service providers. Requires a Bachelor's degree in human services and at least two years of relevant experience, with strong organizational and communication skills.

Qualifications

• Bachelor's degree in human services or related field.
• Minimum two years’ experience in human services or related fields.
• Strong organizational and time management skills with deadline orientation.
• Excellent written and oral communication in English.
• Proficient in Microsoft Office and desktop applications.
• Able to connect with local networks of substance abuse, mental health, and social services.
• Familiar with crisis intervention, boundaries, confidentiality, and trauma-informed care.
• Valid driver's license with good driving history.

Responsibilities

• Assist and monitor veterans' progress or housing retention from entry to discharge.
• Provide intensive site and mobile case management services as needed.
• Identify housing options and assist with securing housing within 90 days of entry.
• Provide linkages to social, mental health, educational, vocational, and substance abuse services.
• Coordinate with other agencies and social service providers.
• Attend weekly and monthly agency, program, and network meetings.
• Monitor cases and complete required documentation in HMIS / Case Worthy.
• Enter and maintain client charts accurately and timely.
• Other related duties to support a 30-client caseload.
