Job summary

Services for the UnderServed is looking for a SSVF Service Coordinator to provide case management for veterans engaged in Rapid Rehousing or Homeless Prevention services. The role involves regular participant engagement, developing Housing Stability Plans, and navigating complex services.

Candidates must have a minimum of three years of case management experience, effective communication skills, and the ability to multitask in a fast-paced environment. This position promotes welfare by engaging veterans and ensuring their access to stable housing and essential services.

Qualifications

  • Minimum of 3 years of case management experience or equivalent lived experience.
  • Ability to effectively communicate with individuals served and team members.
  • Willingness to travel via public transit up to 40% of the workweek.

Responsibilities

  • Provide case management for veterans and their families.
  • Develop and monitor Housing Stability Plans.
  • Engage program participants through regular contact.
  • Coordinate Housing First-oriented services with social service team.
