Job summary

Insight Housing is seeking a Case Manager to support homeless Veteran families with housing stabilization, outreach, and service coordination. Under supervision, you will connect participants to VA benefits, health services, and community resources, while maintaining case files and documenting progress.

You will conduct home visits, manage caseloads, and collaborate with landlords and service partners to promote enduring housing stability and wellbeing.

Qualifications

• High school required; bachelor's degree in social services or related field a plus.
• 1 year of direct support experience with homeless or vulnerable populations required.

Responsibilities

• Coordinate intake and eligibility for new clients and maintain electronic files.
• Provide street and venue-based outreach to homeless Veterans.
• Manage a caseload and deliver housing stabilization services.
• Develop budgets and service plans with each Veteran client.
• Link participants to VA benefits, healthcare, and community resources.
• Maintain confidential records and prepare program reports.
