Job summary

Care Lync is a social services agency helping adults with IDD and autism access essential services to live independently. The Support Coordinator develops individualized care plans and coordinates services across providers to meet each participant's goals.

Responsibilities include assessments, ISP updates, provider selection guidance, confidentiality, and regulatory compliance, with frequent travel to participant homes or communities as required.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain proficiency in company trainings and certifications.
• Maintain state-related Clearances.
• Complete trainings as required by program regulations.
• Valid Driver's License with personal vehicle insurance and current registration.
• Notify supervisor of changes in auto insurance, registration or license status.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or on significant changes.
• Inform participants in selecting providers to support informed decisions.
• Ensure Behavioral and Crisis Plans align with the ISP.
• Report incidents of abuse, neglect, or exploitation immediately.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Consult with supervisors regularly about participant needs and abilities.
• Travel up to 50% to participant locations as needed.
