Job summary

Care Lync is seeking a Service Coordinator / Support Coordinator to develop individualized care plans and coordinate services for adults with IDD/ASD in Pennsylvania. You will broker and monitor services, ensure confidentiality, and work to help clients live independently with community supports.

The role involves assisting with housing, medical, social, vocational services and requires regular collaboration with families and other care coordinators. Travel to clients is 50% of duties.

Qualifications

• Bachelor’s degree required and relevant to program responsibilities.
• Strong organizational and documentation skills.
• Must maintain company‑sponsored training and certifications.

Responsibilities

• Develops individualized care management plans and ISP.
• Coordinate with service providers to meet client needs.
• Monitor and evaluate clinical outcomes and care quality.
• Assist participants in identifying and requesting services.
• Travel to participants as needed (up to 50%).
