Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management for individuals with ID, autism spectrum disorders, or physical disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain proficiency in company sponsored training and certifications.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Successfully complete and maintain training courses as required or amended by program regulations.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
• Inform supervisor immediately when any change in auto insurance policy, automobile registration and/or status of driver's license.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop Person-Centered Service Plans and update as needed.
• Ensure participant choice of providers with information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with ISP.
• Report incidents of abuse, neglect or exploitation immediately.
• Actively coordinate with others essential to care delivery for seamless service.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertifications.
• Regularly confer with supervisor to understand participant needs and limitations.
• Travel to participants as needed (approximately 50%).
