Job summary

Bridgeways is seeking a Jail Transition Case Manager in Everett, WA to help individuals reintegrate after jail release. This full-time role focuses on housing, employment, mental health, and substance use resources, coordinating with law enforcement and social services to reduce recidivism.

The position requires a bachelor’s degree in a related field (master’s preferred) and at least two years of case management or criminal justice experience.

Qualifications

• Bachelor’s degree in social work, psychology, criminal justice, or related field; Master’s degree preferred.
• At least two years’ experience in case management, social services, or criminal justice.

Responsibilities

• Conducts comprehensive assessments of clients’ needs, including risk factors, strengths, and areas requiring support.
• Develops, implements, and updates personalized transition plans addressing housing, employment, education, and health care needs.
• Connects clients with housing, job training, mental health services, and substance abuse treatment, and assists with documentation for benefits.
• Maintains ongoing case management with regular follow-ups and progress monitoring.
• Advocates for clients to ensure access to services from agencies and community organizations.
