GEO Secured Services is seeking a transition case manager to provide services to inmates within one year of release, developing transition plans and connecting them with post-release resources. The role includes individual and group cognitive behavioral treatment sessions as needed and close collaboration with the Continuum of Care team.

Responsibilities include documenting progress, participating in training, and coordinating with state personnel to support reintegration and ensure timely

Qualifications

• High school diploma or equivalent (required).
• Two years of correctional setting experience.
• Ability to prepare written reports and use computers.

Responsibilities

• Participates in designated training for responsibilities.
• Uses GEOtrack to record case notes and progress.
• Develops transition plans with inmates prior to release.
• Conducts individual and group cognitive behavioral treatment sessions as needed.
• Connects inmates with community resources and post-release services.
• Maintains documentation of inmate contacts and reporting data.
