Qualifications

• Prior experience in case management, social work, re-entry services, or a related field.
• Deeply committed to justice-involved individuals' dignity and rights.
• Strong communication skills for one-on-one and group interactions.

Responsibilities

• Conduct comprehensive participant intake assessments.
• Develop goal-oriented Individualized Service Plans (ISPs).
• Provide ongoing case management and support.
• Assist participants in accessing internal and external services.
• Maintain accurate documentation according to protocols.
• Respond to urgent needs and provide crisis support.
• Collaborate with community providers and advocate for participants.
• Support program development by providing feedback.
