GEO Corrections & Detentions is seeking a transition case manager to support inmates within one year of release. You will coordinate treatment, education and post-release planning in partnership with internal teams.

The role emphasizes evidence-based interventions, linkage to community resources, and effective communication with inmates, staff, and external providers to ensure smooth reintegration and reduces recidivism.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in criminal justice, sociology, or a closely related field is desirable.
• Two years of experience in a correctional setting is required.

Responsibilities

• Provide transition case management to inmates within one year of release.
• Record case management notes using GEOtrack system.
• Develop transition plans and connect inmates to community resources before release.
• Conduct individual and group cognitive behavioral treatment sessions as needed.
• Coordinate with community providers to support successful reintegration.
