Job summary

Horizon Behavioral Health in Lynchburg, VA is seeking a Case Manager for a grant-funded reentry program. You will manage a caseload of justice-involved adults with substance use disorders, coordinating assessments, planning, and service delivery.

The role requires a bachelor’s degree in a human services field and at least one year of related experience, with strong collaboration skills across agencies to support community reintegration and MAT continuity.

Qualifications

• Bachelor’s degree in a human services field or equivalent education demonstrating required knowledge and abilities.
• Minimum of one year of relevant mental health, developmental disability, and/or substance use work experience with the identified population.

Responsibilities

• Manage an assigned caseload of justice-involved adults with SUD and co-occurring conditions.
• Develop individualized reentry and service plans and coordinate evidence-based treatments.
• Collaborate with correction staff, providers, and community partners to ensure continuity of care and barriers removal.
