Qualifications

• Bachelor's degree in Social Work or related field or equivalent experience.
• Experience with justice-involved individuals and behavioral health populations.
• Knowledge of court systems, probation, parole, and reentry services.
• Ability to work independently in field-based environments and manage multiple caseloads.
• Valid Arizona driver's license and reliable transportation.
• Must pass background checks and fingerprint clearance.

Responsibilities

• Justice System Coordination: communicate with correctional facilities, probation, parole, and courts and coordinate reentry services before/after release.
• Case Management & Care Coordination: assess needs, develop service plans, connect to housing, employment, behavioral health, and medical resources.
• Reentry & Stability Support: assist clients transitioning from incarceration and address barriers to service engagement.
• Documentation & Compliance: maintain timely documentation and ensure HIPAA, 42 CFR Part 2, ADHS, AHCCCS standards.
• Community & Facility Engagement: travel within assigned regions, build relationships with reentry coordinators and facility staff.
