Job summary

Bridgeways Everett is seeking a Jail Transition Case Manager to support individuals reentering the community after incarceration. You will assess needs, connect clients with housing, employment, mental health, and substance use resources, and coordinate with law enforcement, social services, and community partners.

Qualifications

• Bachelor’s degree in social work, psychology, criminal justice, or related field (Master’s preferred) or equivalent experience.
• Minimum two years’ experience in case management, social services, or criminal justice (criminal justice system preferred).
• Must be authorized to work in the United States and able to pass a background check.

Responsibilities

• Conduct comprehensive assessments of clients’ needs, risks, and strengths.
• Develop and update personalized transition plans addressing housing, employment, education, and health care.
• Connect clients with housing, job training, mental health services, and benefits; assist with documentation.
• Maintain ongoing case management with follow-ups and progress monitoring; adjust plans as needed.
• Provide crisis intervention and advocate for services across agencies and community organizations.
• Facilitate life skills, job readiness, and personal development groups.
• Protect client health information per HIPAA; document in EMR accurately.
• Represent Bridgeways in community activities and work groups.
