Enhabit Home Health & Hospice invites applications for a role focused on ensuring accurate patient information review and compliant documentation across start of care, recertification, and evaluations. You will collaborate with branch leadership to educate Licensed Professionals and deliver targeted in-depth training as needed.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy (RN/PT).
• Valid license as required by state regulations; active license to practice.
• Minimum of one year of professional experience.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification required.
• Valid state driver's license and automobile liability insurance required.
• Preferred experience with packet review or quality improvement.
• Preferred experience with coding.
• Basic technology skills including use of mobile devices.
• Experience with Homecare Homebase (HCHB) preferred.
• In-office attendance essential for supervision and collaboration.

Responsibilities

• Review and approve patient information submitted during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation requirements to meet regulatory standards.
• Provide one-on-one training to LPs when necessary.
• Promote quality patient care and maintain strong relationships with referral sources, patients, caregivers and staff.
