Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Temple, OK. The role supports the Physical Therapist with screenings, treatment plans, and direct patient care to residents in a structured care setting.

Qualifications include an accredited associate degree in physical therapy and an active PTA license in the practice states. Ongoing education and CMS-compliant documentation in NetHealth EMR are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the Physical Therapist.
• Assist in establishing target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments including strength, mobility, balance, gait, and ROM work.
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents' response and consult with PT if modification is needed.
• Assist in scheduling with PT and other team members.
• Train residents, family, and appropriate staff in equipment use.
• Participate in resident, family, and staff education.
• Ensure documentation meets CMS expectations in NetHealth EMR.
• Report to work on time and maintain productivity during shifts.
