Job summary

Evolve Therapy Services, LLC invites applications for a Physical Therapy Assistant to join our in-house Skilled Nursing Facility team in Temple, OK. You will work under the Physical Therapist to deliver patient-centered therapy and support resident mobility, strength, and independence.

The role requires an Associate’s degree, an active PTA license, ongoing continuing education, and a commitment to CMS NetHealth documentation and teamwork with residents, families, and staff.

Qualifications

• Associate’s degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatment focusing on strength, mobility, balance, gait, ROM and related modalities.
• Assist in selecting techniques/media and adapt activities per resident performance.
• Monitor residents’ response to intervention and modify as needed with guidance from the Physical Therapist.
• Assist in scheduling in coordination with the Physical Therapist and team members.
• Adhere to schedules and modify as appropriate for treatment regimens.
• Train residents, families, and staff in equipment use and safety.
• Participate in resident, family, and staff education.
• Ensure timely, compliant documentation in CMS CMS NetHealth EMR systems.
