Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and link clients to housing support resources.
• Complete program assessments (e.g., VI-SPDAT) and document findings.
• Monitor progress and conduct 90-day reviews with clients.
• Maintain accurate case files and confidentiality per policies.
• Comply with HIPAA and program guidelines.
