Job summary

National Health Foundation in Los Angeles seeks a Housing Navigator to assist participants in housing crises through case management and housing placement. You will coordinate with landlords, housing authorities, and community partners to secure permanent housing and support services, while developing individualized housing plans and budgets.

The role requires field-based work, strong collaboration with interdisciplinary teams, and a commitment to ending homelessness.

Qualifications

• BA or AS degree with 3+ years in social services preferred.
• Experience working with unhoused individuals is a plus.
• All positions require background check, LIVE scan and TB test.

Responsibilities

• Conduct initial meetings and assessments to evaluate housing needs, history, barriers, and available services.
• Review intake assessments and connect participants to resources for permanent housing and subsidies.
• Develop an Individual Action Plan (IAP) with goals and link services to address those goals.
• Provide housing stabilization services, including money management, life skills, and lease understanding.
• Cultivate relationships with landlords and housing providers to expand housing options.
• Enter data in required databases and maintain guest files and eligibility documents.
• Coordinate field-based services at identified motels and interim housing locations.
• Follow NHF policies and procedures and participate in care coordination meetings.
